Cardiac Drugs & Nursing Responsibilities ❤️🩹
Calling all NORCET, AIIMS & RRB aspirants! Cardiac pharmacology is a high-scoring, frequently repeated topic. Master these facts! 🎯
🔹 Digoxin (Cardiac Glycoside)
Used in: Heart failure, Atrial fibrillation
⚠️ ALWAYS check apical pulse for 1 full minute before administration
Hold dose if pulse <60 bpm (adult) or <90–110 bpm (infant) — notify physician
Therapeutic level: 0.5–2 ng/mL
Toxicity signs: Nausea, vomiting, visual disturbances (yellow-green halos), bradycardia
Antidote: Digoxin Immune Fab (Digibind)
🔹 Common Antiarrhythmics
Amiodarone — broad-spectrum, used in V-fib/V-tach
Lidocaine — ventricular arrhythmias
Adenosine — SVT (given as rapid IV push, very short half-life)
🔹 Anticoagulants (Frequently Confused Pair) ⚡
Heparin: Immediate action, IV/SC, monitored via aPTT, antidote = Protamine sulfate
Warfarin: Delayed action, oral, monitored via PT/INR, antidote = Vitamin K
🔹 Thrombolytics
Streptokinase, Alteplase — used in MI/stroke within the “golden hour”
Contraindicated in active bleeding, recent surgery, stroke history
🔹 High-Yield PYQ Facts 📊 ✅ Drug of choice for cardiac arrest → Epinephrine ✅ First-line drug for MI chest pain → Morphine (also given: Aspirin, Nitrates, Oxygen — “MONA”) ✅ Beta-blockers (e.g., Metoprolol) — contraindicated in bradycardia & asthma ✅ Nitroglycerin — check BP before administration (causes hypotension) | 1 853 |
