• ↓ انخفاض ضغط الدم الانقباضي كان أكبر مع العلاج الرباعي
• ↓ تحسن أفضل بضغط الـ 24 ساعة (ABPM)
• 💎أمان متشابه بين المجموعتين
• ❌لا زيادة في الأعراض الجانبية الخطيرة

تضرب ال muscle و تسوي Rhabdomyolysis و AKI لذلك كلش نخاف ع الrenal و نمشي كبل fluid
تضرب GIT و تسوي stress ulcer
ممكن تسوي Fit و burns كذلك
و ممكن حتى # تسوي
📌لذلك العلاج حسب شدة الاعراض و وضع المريض
📌المهم مريضنا انطيناه:

ECG interpretation skill is a lifelong journey, there is always something new to learn. I used to think that you can definitely diagnose dextrocardia using ECG alone if the following things come together:
- global negativity in lead I (P, QRS, T)
- global positivity in aVR (P, QRS, T)
- poor R wave progression
I used to educate my colleagues in this way.
However, a friend sent this ECG to me, it matches the above criteria, but I was surprised this patient did not have dextrocardia!
He had poor R wave progression due to an old anterior MI, while the negative lead I and positive lead aVR were due to the lead reversal.
I learned that you can never be 100% certain whether there is dextrocardia or not using ECG alone, it is not enough. We still need history, examination, and even maybe CXR.
So whenever you see, global negativity in lead I, global positivity in lead aVR and poor R wave progression, this could be dextrocardia or lead reversal, we can only confirm it 100% by examination and CXR. | 1 133 |
