🔥 A Story of Unrelenting Breast Cancer 🔥 A 40-year-old woman found herself in a medical mystery after being referred to a general surgeon for severe erythema and swelling in her left breast. Initially, it seemed like a simple case of mastitis with an abscess, leading to an incision and drainage procedure. However, things took a dark turn. Tissue samples from the procedure indicated fat necrosis. Despite a course of antibiotics and anti-inflammatory drugs, her condition worsened. After a month of treatment, her symptoms persisted. The next round of tests—routine blood work, chest X-rays, ultrasonography, and mammography—offered few clues, except for a high erythrocyte sedimentation rate (51mm/h). This suggested ongoing inflammation, but there were no masses or microcalcifications. Mammography revealed an infiltrative pattern, hinting at more severe conditions like carcinoma or inflammatory changes. One month later, her symptoms escalated with skin thickening and new skin lesions. A visit to the dermatologist revealed troubling congestion, inflammation, and violaceous papulovesicular lesions across her left breast. Her right breast also showed signs of erythema. Tenderness was noted during bimanual examination, but no lumps were palpable. The histopathological examination of the left breast showed malignant neoplasms involving the dermal lymphatics, pointing toward a comedocarcinoma and invasive ductal carcinoma. The right breast's biopsy revealed lymphatic channels filled with atypical cells with hyperchromatic nuclei. The final diagnosis was inflammatory breast carcinoma with skin metastases. With this dire diagnosis, the patient was referred to oncology for further treatment.

The snake was identified as a Levatine viper (Macrovipera lebetian). Physical exam showed a swollen penis with hemorrhagic bullae at the puncture sites. Lab tests revealed coagulopathy. The patient received anti-snake venom with no adverse effects. After 36 hours, the coagulopathy resolved, and he was discharged. The swelling subsided in 4 days, with necrotic tissue formation at the puncture sites. The patient fully recovered in 2 weeks.
