• Bat-wing perihilar shadowing

• Orthopnoea (cannot lie flat)
• RR = 31/min
• BP = 90 mmHg

❓ ليش ممكن يكون الخلط خطير جدًا؟
🔬 أغلب مضادات الفطريات الفموية من مجموعة الـ Azoles مثل:
Fluconazole
Itraconazole
Ketoconazole
هاي الأدوية بتثبط إنزيمات الكبد CYP450 (خصوصًا CYP3A4).
💊 لو المريض كان ياخد مهدئات مثل:
Midazolam
Diazepam
Alprazolam
⬅️ بصير استقلاب المهدئ أبطأ
⬅️ يرتفع تركيزه بالدم
⬅️ يزداد التثبيط العصبي
⬅️ احتمال كبت تنفس / غيبوبة
⚠️ أخطر مثال: Midazolam الفموي مع azoles (تداخل شديد وممكن يكون مميت).

المشكلة مش إنهم الاثنين بنوّموا…
المشكلة إن مضاد الفطريات برفع مستوى المهدئ بشكل خطير داخل الجسم.
Normal ECG + recurrent episodes → Holter monitor
