A- interstitial keratitis

🔆Rhinophyma :
Is a subtype of #rosacea, which is a skin condition that primarily affects the face.
Rhinophyma specifically refers to the #enlargement_and_thickening_of_the_nose due to #excess_tissue_growth.
It is characterized by a #bulbous and #red appearance of the nose, often accompanied by #prominent_pores and #visible_blood_vessels.
Rhinophyma is thought to be primarily caused by untreated or poorly managed rosacea, although the exact underlying mechanisms are not fully understood.
It tends to occur #more frequently in #men than women and typically develops in individuals who are over the age of #40.

🎀 الجلدية نقدر نقسمها بكذا طريقة :
☆نقدر نقسمها على حسب السبب يعني مثلا :
[ infectious, inflammatory, systemic ]
والinfectious تبقى >>
[ fungal, bacterial, viral, parasitic ]
☆أو حسب شكل ال lesion هل هو: . discoid
ولا plaques
ولا pleomorphic
ولا .....الخ
☆أو حسب مكان الlesion مثلا في
الtrunk
ولا الface
ولا ....الخ
☆أو حسب الأعراض مثلا itchy ولا لأ ، معاه حرارة ولا لأ..... الخ
طبعا لإتساع الفروع هاتكلم عن الحاجات الشائعة ، واللي بتيجي كتير في الامتحانات .

⭕️ هنبدأ ونقول إن الجلد بيتكون من 3 طبقات :
[Epidermis, dermis, subcutaneous tissue]
⭕️ وبالنسبة لأنواع الlesion ممكن نقول ببساطة إن في :
■ macule & patch :
ودي بتبقى
flat & non-palpable ،
>> الmacule أصغر من 1 سم
>> والpatch أكبر من 1 سم
■ Papule & Plaque :
ودول مرتفعين عن سطح الجلد و palpable ،
>> الpapule أصغر من 1 سم
>> والplaque أكبر من 1 سم
■ Vesicle and bulla :
ودول مرتفعين عن سطح الجلد برضو بس
fluid-filled
>> والvesicle أصغر من 1 سم
>> والbulla أكبر من 1 سم
>>ولو اللي جوه الvesicle صديد يبقى اسمه
pustule
■ولو الlesion عميق يعني داخل في الdermis وربما الSC tissue
يبقى

(وده يتحس بال palpation مش بالمقاسات) لأنه من بره يبقى نفس شكل الpapule
■ أما لو عميق برضو بس جواه fluid
(وده برضو يتحس بال palpation)

⚠️ فأنا عليا إذا شفت أي skin lesion :
⬅ أبتدي الأول أوصفه :
📍نوعه إيه؟
(من الأنواع اللي قلناها فوق)
📍واحده بس ولا كتير؟
📍ولو bilateral هل هو symmetrical في الجهتين؟
📍 لونه إيه ؟
📍شكله إيه؟
📍 سطحه شكله إيه؟
يعني مثلا rough زي >>
wart
ولا ulcerated زي >>
squamous cell carcinoma
ولا excoriated يعني عليه علامات هرش زي >>
eczema
ولا عليه crustation أو scales؟
📍شكل الborder؟ محددة ولا لأ؟ active ولا لأ؟
📍 وكمان بنشوف موجود فين؟
وهنا لازم أفكركم إن علينا نفحص الأماكن المستخبية حتى لو المريض ما اشتكاش ،زي مثلا :
>> الthigh من الداخل
>> وتحت الfemale breast
>> وفي منطقة الgenitalia
>> والgluteal cleft
>> وكمان الmucous membranes
⬅ بعدها أعمله palpation وأشوف هل tender؟
وأشوف الconsistency والعمق والحرارة والحركة mobility .
💡في امتحانات الslides بيجيب لك غالبا keyword عن الlesion في سيناريو الحالة وهنا هنركز عليها إن شاء الله وهاحطها بين أربع أقواس (())، أما الspot diagnosis بيكون غالبا محصور في الحاجات الواضحة زي الشمس .
