🔹Clinical Staging of CLL #باطنة

🔵 Anemia and Thrombocytopenia ?? في ال CML , CLL 🔹 CML :- Due to Hypersplenism 🔹 CLL Due to Autoimmune Hemolytic anemia and Thrombocytopenia. ( Evan's syndrome) . #باطنة

🛑 Splenectomy:- نحتاجه في الحالات اللي يحصل فيها Spleenomegaly. لكن ! سبب ال Spleenomegaly يا اما يكون ; - Destruction of cells. or - Extramedullary Hematopoisis. 🟡🟡 منطقياا لو كان عندي Cell Abnormal بيكون ال Spleen يكسر الخلايا .. فاعمل Splenectomy عشان يقل التكسير واحسن الوضع . Indications of spleenectomy:- 1- Thalasemea 2- Hereditary spherocytosis or Elliptiocytosis 3- Autoimmune Hemolytic anemia , Autoimmune Thrombocytopenia ( Evan's syndrome ) 4- Idiopathic thrombocytopenia 5- Pancytopenia secondary to Hypersplenism ( ex: Malaria ) 6- Accelerated phase of CML 🟡🟡 بالمقابل لو كان عندي Bone marrow failure + Spleenomegaly. المفروض ما اعملش Spleenectomy مثل :- 1- Aplastic anemia 2- Megaloblastic anemia 3- Mylofibrosis 4- .. #باطنة

🛑 وجود ال Philadelphia chromosome :- BCR -ABL اكتشفوا مؤخرا في جامعة في فيليدلفيا انو:- The BCR gene is normally on chromosome number 22. The ABL gene is normally on chromosome number 9. The BCR-ABL mutation happens when pieces of BCR and ABL genes break off and switch places. يحصل Translocation ويتبادلوا . This gene then produces the BCR-ABL protein, which is the type of protein called a tyrosine kinase. This protein causes CML cells to grow and divide out of control. وما يحصلش لها Apoptosis . و لحسن الحظ اكتشفوا له علاج :- Tyrosine Kinase inhibitors. مثل:- imatinib , axitinib , nilotinib وغيرها ... اكتشافهم لوجود الجين و العلاج اللي يوقف عمل ال Tyrosine Kinase يعتبر Good Prognosis للمرضى اللي عندهم CML لانو نقدر نسيطر على المرض . ☹️☹️☹️☹️ لكن !!! لو كانت AML او ALL و حصلوا فيها ال Philadelphia chromosome هذا يعتبر Bad prognosis !!!! ليش. ؟! لانو وجوده معناته انه كانت Chronic Myloid luekemia. و حصل Acceleration و بعدها Acute leukemia on top of Chronic وبهذا كانت زمان تستجيب للعلاج والان صارت ما تستجيب... ولازم نحولها لعلاجات ال Acute. #باطنة

🔹Therfore ; In the treatments of ALL , نضيف له Intrathecal methotrexate وهذا زي اللي عطوه للأطفال اللي بمستشفى الكويت 🤐 #باطنة #أطفال

بالنسبة لل Acute leukemia بشكل عام 🔸 infiltrations is more with ALL due to it's small size ; ( HSM , lymphadenopathy, Meningitis, salivary glands " parotid " ) 🔶 Special for AML : According to FAB :- M3 [ acute promylocytic Leukemia ] >>> Auer rods >> responds to ATRA >> Good Prognosis >> DIC may occur in stage M3 >> infiltrations of the Gum [ Gum hypertrophy] in M4 , M5 #باطنة

🛑 Acute lymphoblastic leukemia ( ALL) من اسمها :- 🔺1) SmALL >>> حجمها صغير ولأن حجمها صغير :- 🔺2) more in Children ( 1-5 ) years 🔺3) More infiltration and metastasis to other Tissue like Brain and testis - more HSM , Lymphadenopathy - may do Salivary gland swellings وبالتالي يصير علاجها 🔺4) Needs intensive treatment for long time , with high recurrence rate . ( induction - consalidation- Mantinance) ولانها صغيرة وتعبر Pass 🔺5) Stain with PAS stain . #باطنة

G6PD is intravascular heamolysis Due to lack of protection aganist the oxidative stress aganist RBCs X-linked Recessive . On blood Film :- ال Oxidative stress بيهاجم خلايا الدم ... بتكون زي المأكولة Bite cells او يتكون فيها Heniz bodies .. ولازم نبتعد عن العوامل اللي بتزيد التكسر . #باطنة #أطفال

🟡Hereditary Spherocytosis:- Autosomal Dominant . Defect in mambrane proteins ( Spectrins , Ankyrins) >> abnormal shape >> engulfed by spleen [ Extravascular Hemolysis] في هذي الحالة يكون :- Splenectomy corrects the anemia .. وفي هذي المرض تكون خلايا الدم Fragile .. تتكسر بسهولة لانو ال Mambrane ناقص البروتينات لهذا نأكد المرض بالفحص :- Osmotic fragility test وتكون الخلايا بال smear:- Spherocytes [ Hyperchromia ] #باطنة

In Megaloblastic anemia:- - to know weither it is Vitamin B12 or B9 .. ↗️ Homocysteine in Both deficiencies ↗️ ( MMA ) Methylmalonic acid ( in Vitamin B12 ) deficiency 🔵 Note :- When you treat the patient you must give him both Vitamin B9 + B12 لان B9 يعتمد في عمله على ال B12 . وعشان يتحول Dihydrofolate >> tetrahydrofloate يحتاج إلى B12 ولو عالجنا ب فيتامين B9 فقط و عندنا نقص في B12 ندخل المريض في Folate Trap . #باطنة

⚪️ Sidroblastic anemia:- ( Defect in protoporphyrin synthesis) We need Protoporphyrin + Irom to form= Haemoglobin هنا ما عاد عيتكونش ال Protoporphyrin ! والسبب؟ الانزيم اللي اسمه " Aminolevulinic acid synthase " ما يشتغل سوا وهو يحتاج الى Vitamin B6 ( (pyridoxine) اللي بيصير انو يظل ال Iron متراكم بال Cells ويكون Sidroblastic ring في ال BM 🔹 Causes ? 1- Hereditary [ X- linked ] 2- Deficiency of Vitamin B6 3- Lead Poisoning 4- Alcoholism 5- Drugs like Anti- TB : Isoniazid. 🔹 Findings:- - Anemia menifestations - High serum iron , High serum ferritin , high transferrin , low TIBC - the presence of bone marrow ring sideroblasts, erythroblasts with coarse blue granules at Perls staining " Pappenheimer bodies " 🔹 treatment:- - Vitamin B6 - Blood transfusion #أطفال #باطنة
