| 3 | Diagnóstico de una persona que no puede diferenciar su imaginación de los hechos objetivos.
La incapacidad de distinguir la realidad de la imaginación, la fantasía o los propios pensamientos no es una enfermedad única en sí misma, sino que clínicamente se conoce como un estado de psicosis o una pérdida del juicio de realidad.
A nivel clínico y según el tipo de desconexión o comportamiento, esta condición puede recibir diferentes nombres o formar parte de distintos diagnósticos:
• Psicosis: Es el término general para describir un episodio o estado mental en el que la persona experimenta una ruptura con la realidad. Durante un brote psicótico, la mente confunde estímulos imaginarios o internos (como voces o visiones) con hechos reales del entorno.
• Trastorno delirante o Esquizofrenia: Son diagnósticos específicos de salud mental donde el síntoma principal es la convicción absoluta de ideas falsas o imaginadas (delirios) o la percepción de cosas que no existen (alucinaciones), sin que la persona pueda notar la diferencia.
• Mitomanía o Pseudología fantástica: Si la persona no confunde percepciones sensoriales sino que construye historias inventadas, fantasías o mentiras complejas y termina creyéndoselas como hechos reales, el diagnóstico suele orientarse hacia este trastorno de la conducta.
• Ensoñación desadaptativa (o excesiva): Es una condición en la que la persona pasa horas inmersa en fantasías extremadamente vívidas. Aunque en este caso la persona suele saber de manera racional que lo que imagina no es real, la intensidad de la fantasía es tan alta que puede absorber por completo su vida cotidiana, actuando como un escape de la realidad. | 310 |
