تأثيرة ع thyroid gland
Amidarone can cause hypothrodisim in 30% of cases , 15 % hyperthyroidism
اولا هايبو ثايرودسم hypothyroidism:

هذا سهلة
لان الاميدارون بي iodide هواية تقريبا ٧٥ مغم بكل ٢٠٠ مغم كوردارون هذا الايودايد الزايد يسوي wolf chaikoff effect (its auto regulatory phenomenon where thyroxine formation is inhibited due to high circulatory level of iodide)
المهم هو يمنع التحول T4الى T3 (مستواه يقل فهذا يرسل رسالة استغاثة ل pituitary ) بالتالي المحصلة النهائية TSHيصعد وال T3 يقل و ال T4 يزداد او يبقى طبيعي
-اذا صار عندك هايبو ثايرودزم من ورا الاميدارون اذا المريض يكدر يتركة خل يتركة (اذا مياثر ع arrhythmia) اما اذا المريض يحتاجة ف نضيف للمريض thyroxine
ثانيا:
Hyperthyroidism:

المهم اكو نوعين من الميكانزم وكلش مهم تعرفة لان العلاجات تختلف :
Type1:
الاميدارون يشتغل dirct on follicular cells —-> يدمرة ويسبب release of thyroid hormones to the circulation
كذلك الاميدارون يسبب destructive thyroditis) فينفرز ال t3-t4 برة فيصير thyrotoxicosis
هذا النوع المريض
🫶🏼ماعندة انتفاخ بالغدة، ويصير عندة الم بالركبة neck pain ،
❤️🔥واهم شي انو بالدوبلر يبين انو decrease blood flow to the thyroid هذا اهم فرق حتى تفرقة من نوع الثاني ، iL6 يزداد بهالنوع
وعلاجة prednisone 40 mg
Type 2:
اهنا الاميدارون يسرع من انتاج T3 ,T4 بسبب كثرة ال iodide الي بي (يسمى ب jod-basedow effect)اني هم محافظ الاسم لامانة ف مو تعوف الميكانزم وتصفن ع اسم ،
وهذا الميكانزم يصير بمرضى الي عدهم مشاكل مسبقة بغدة الدرقية مثل goitre ،
وبالدوبلر يزداد اندفاق الدم الى الغدة الدرقية ، و thyroid antibodies +ve
والعلاج هو : carbimazole 40 mg , or , lithium carbonate or surgery ,,,,,
-ويفضل المريض الي صار بي النوعين من ال thyrotoxicosis انو يغير الاميدارون لغير علاج واذا ماكو بديل يبدي بالعلاج لفترة حتى يحضر المريض لعملية thyrodectomy total | 1 578 |

Contraindications in🚨🚨🚨🚨🚨
1-Hyperkalimia
2-digoxin induce VT
3-Torsades de point ( prolonged QT interval)
🔹side defect of this drug جدا مهم نعرفها
1️⃣Pulmonary toxicity(fibrosis)
2️⃣Thyroid dysfunction (both hypo- and hyperthyroidism)
3️⃣Liver toxicity
4️⃣Corneal deposits
5️⃣Skin discoloration (blue-gray)
6️⃣Photosensitivity
💌لذلك ضروي متابعه المريض
ونسوي follow up للمريض عن طريق

ندزه بالبدايه قبل العلاج ك base line وبعدها
كل 3-6 month

🔴1- Cardiac cause 👉👉dependent edema
وبما انو اغلب مرضى HF يكونون قليلين الحركه راح تصير
Leg edema 👉👉in setting patient
Sacral edema 👉👉in lyning patient
بعدها من تزداد كميه الfluid راح يورم كل الجسم ويصيرascitis
🔴2-hepatic cause 👉portal HT 👉start as ascitis
لان يزداد الhydrostatic pressure in portal vien يطلع الfluid
بعدها نتيجه خروج الfluid راح تصير hypovolimia لذلك راح يتحفز ال
RAAs system 👉👉highly aldosterone 👉👉sodium and water retention 👉👉generalized edema
🔴3- Renal cause 👉Nephrotic cause👉hypoprotinemia
المريض حركته طبيعيه
فقط خلال النوم راح يبلش تجمع السوائل حول العين
Peri orbital puffiness
بعدها يتطور ويصير generalized
ايضا نتيجه RAAS✅✅

عاده يصير بعد يومين الى اسبوعي

Hemorrhagic transformation of an ischemic infarct occurs within 2–14 days postictus, usually within the first week. ✅✅✅
It is more commonly seen following cardioembolic strokes and is more likely with larger infarct size. ☠️☠️☠️☠️
Hemorrhagic transformation is also more likely following administration of tissue plasminogen activator (tPA) 🔥🔥🔥🔥🔥in patients whose noncontrast computed tomography (CT) scans demonstrate areas of hypodensity.

| 9 |  +2اكو تداخل بين Ciprofloxacin و alkalizing agents المستخدمة بتخفيف اعراض التهابات المسالك البولية
استخدام Ciprofloxacin مع ال alkalizing agents يسبب crystalluria يعني رمل باليورن
لان Ciprofloxacin بالوسط القاعدي يترسب ويسوي crystalluria بالحالب او بالمثانة
ممكن تضهر على شكل
Ca oxalate
Uric acide
وهنا بدل مانحل مشكله المريض ندخله بحالة ثانية وهي تكون كرستال والي ممكن تتطور الى حصوات
الحل:
✅لازم نتجنب alkalizing agents خلال فترة استعمال Ciprofloxacin
بعض الدراسات تفضل الفصل بينهم اربع ساعات
وبكل الاحوال من ياخذ المريض Ciprofloxacin ننصحه يكثر من شرب الماء لان يساعد بدفع والتخلص من البكتيريا وايضا يساعد بمنع ترسب السبروفلوكساسين✅ | 3 347 |

Steroids & Mannitol in Ischemic Stroke: What Does the Evidence Say?
1. Corticosteroids in Ischemic Stroke
■Traditional Practice: Steroids were historically considered to reduce cerebral edema in ischemic stroke.
Current Evidence:
■No proven benefit in improving outcomes.
■The Cochrane Review (2009) found no reduction in mortality or disability and potential harm from hyperglycemia and infections.
■AHA/ASA Guidelines (2021): Do not recommend corticosteroids for ischemic stroke-related cerebral edema.
✅ Take-Home: Steroids should not be used for routine management of ischemic stroke.
2. Mannitol in Ischemic Stroke
■Traditional Practice: Mannitol was widely used for stroke-related brain swelling.
Current Evidence:
■May provide temporary reduction in intracranial pressure (ICP), but does not improve long-term outcomes.
■INTERACT trial & other studies: Aggressive osmotherapy does not significantly improve functional recovery.
■AHA/ASA Guidelines: Reserve mannitol (or hypertonic saline) for patients with severe cerebral edema causing herniation or neurological deterioration.
✅ Take-Home: Mannitol is not routinely recommended in ischemic stroke but may be used in malignant cerebral edema with herniation risk.
Bottomline:
❌ Steroids - No role in ischemic stroke.
⚠️ Mannitol - Limited role, only for life-threatening cerebral edema.
