-اول حاجة: لو كان موجود فى التحليل pus cells يعنى موجود عدوى بكتيرية
*لو كان اقل من 15 مش ضرورى توصفله مضاد حيوى ،تكتفى بمضاد للبكتيريا زى:-
ال nitrofurantion اللى موجود فى uvamin و nitrofuran
*أما اذا كان اكبر من 30 يبقا لازم تسأل المريض هل فيه حرقان اثناء التبول او الم؟...
-تانى حاجة هتبص عليها هى وجود خلايا اسمها casts فى التحليل
لو لقيت التهاب فى الكليتين (pyelonephritis)
او لو الالتها تحت الكليتين (cystitis)
-لو فى حالة ال cystitis توصفله مضاد حيوى سيبروفلوكساسين مثلا لمدة ٣ أيام فقط ميكملش الشريط للاخر
-أما لو فى حالة ال pyelonephritis توصفله ٧ بالسيبرو لمدة ١٤ يوم بس بالمضادات الثانية زى سبترين

لازم تستبعد إلتهاب اللوز في حالة طفل بيشتكي من ترجيع ووجع في البطن، حالات كتير بتيجي الاستقبال أو العيادة بشكوي الترجيع ووجع البطن وربما الاسهال والطبيب بيتعامل علي إنها نزلة معوية حادة بدون فحص اللوز والحلق بخافض اللسان لاستبعاد التهاب اللوزتين

من الأساسيات إن أي طفل ظاهر عليه أي أعراض بشكل عام، إننا نفصح الحلق قبل أي إجراء بالتزامن مع باقي العلامات الحيوية، فحص الحلق هيخليك تستبعد أو تشخص أمراض كتير زي sinusitis لو فيه posterior nasal discharge أو ال tonsilitis وال pharyngitis لو فيه صديد علي الوز والحلق
وصف العلاج المناسب يتم بناء علي المرض المُشخّص لا الأعراض،فلو عالجت أعراض زي الترجيع ووجع البطن وأهملت المرض الأساسي اللي هو التهاب الحلق واللوز الطفل مش هيخف، لازم نعطي مضاد حيوي مناسب لبكتيريا ال gram +ve زي مجموعات ampicillin salbactam أو ال amoxcillin clavulinic acid أو ال macrolides أو ال first generation cephalosporines ،مع مضادات التهاب وتورم مع مطهر معوي( الافرازات الصديدية اللي بتنزل المعدة بتسبب 2ry gastroenteritis فمفيش مانع نعطي مطهر معوي)
لو الطفل حرارته أكتر من ٣٩ علق محلول وريدي paracetamol حسب الوزن (١سم/ كجم) أو اعطي لبوس (الشراب مش هيشتغل طالما درجة الحرارة عالية لازم حقن أو لبوس أو كمادات) مع pansol حسب الوزن وحط عليه حاجة للترجيع آمنة في الأطفال زي danset وبلاش برمبران للأطفال والحوامل والمرضعات (حسب للوزن 1 سم لكل 10 كجم ما عدا المسكنات ومضادات التقلص بتبقي 1/2 سم لكل 10 كجم) وممكن تحط حاجة للتقلص وحاجة للمعدة زي أنتودين.
