Hypothyroidism and coronary artery disease CAD : هذا الموضوع كلما تقرء بيه اكثر، راح تحصل معلومات أكثر ، و هو جداً لطيف و كمن . ال heart performance يعتمد على ال balance between demand of the heart and blood supply, ف اذا قل ال supply او زاد ال demand, دخلنا بموضوع ال ischaemic heart disease و اعراضة المعروفة عند الكل . بحالات ال هايبوثايرود الي يصير هو الأتي : Marked coronary artery disease but there is context marked decrement in cardiac demand . ف بالرغم ان المريض عندة CAD, لكن ما عندة IHD, لأن ال balance between demand and supply is still there . فمن تريد تنطي ثايروكسين لهذا البيشنت، راح تزيد ال كارديك ديماند، و لكن ال supply يبقى كما هو، فمباشرة يدخل المريض ب IHD . لذلك من تريد تنطي T4 ل هؤلاء البيشنتيه، لازم تنطي وياه anti anginal, و يفضل الانديرال ، لحد ما تبدي تعالج ال CAD ك اي بيشنت أخر . طبعاً الموضوع أعمق من هچي بس هذا المهم . Summary : hypothyroidism patients always have context CAD but it is asymptomatic .

Tuberous sclerosis is a rare disease that causes tumors, or growths, in the brain and other organs. These growths can occur in the skin, kidneys, eyes, heart, or lungs. They are usually benign (non-cancerous). The first signs of tuberous sclerosis may occur at birth. Other people develop symptoms over time. Some of the first signs are seizures and spots on the skin. Some people who have tuberous sclerosis may have learning problems or seizures that are hard to control.

Look to this epigastric mass which is bilobulated in 36 day infants . It is seen more obviously at time of feeding with projectile non bilious vomiting . It is a case of congenital hypertrophic pyloric stenosis CHPS .

صباح الخير شنو الفرق بين ال Rheumatoid arthritis وبين الrheumatic fever (rheumatic arthritis ) اول شي نجي لل cause Rheumatic fever سببها autoimmune reaction to G A beta hemolytic streptococcus ،واكثر شي يتعرضولها non-treated cases او inappropriately treated Rheumatoid arthritis السبب مالته unknown واكو اله factor منهن autoimmune ويدخل genetic بزيادة احتمالية التعرض له ثاني شي ال clinical picture 1_age : RF ,young age واكثر شي (5-15 سنة ) , tonsillitis بالبكتيريا المعروفة عادة بعد الاصابة ب ,RA عادة female بعمر اربعين سنة 2_من ناحية الاعراض RF acute ,asymmetrical migratory ,dramatic response to aspirin وعادة تكون knee RA ,chronic, pain ,swelling, morning stiffness ,symmetrical ,less response to aspirin واكثر شي hand ,wrist,knees,ankles feet affected X-ray normal وماكو rheumatoid factor ب RF Aso titer positive #welcome_to_rheumatology #A
