It is a case of tuberous sclerosis TS.
TS is neurocutaneous syndrome.
CNS manifestations include : seizure, mental retardation, infantile spasm.
Cutaneous manifestations include : adenoma sebaceoum, Ash leaf (hypo pigmented lesions as in the 1st picture), peri-ungual fibroma (in the 2nd picture).
Emergency presentation : hypovolaemic shock due to rupture of renal angiolipomyoma.

The pt is old age female with hx of HT and DM, both are uncontrolled.
Presented with weakness since 2 days.
O/E : lethargic but responsive, pulse is detected at carotid artery only.
What is the 1st action??

The Push-pull effect :
Hypokalaemia creates the illusion that the T wave is “pushed down”, with resultant T-wave flattening/inversion, ST depression, and prominent U waves
In hyperkalaemia, the T wave is “pulled upwards”, creating tall “tented” T waves, and stretching the remainder of the ECG to cause P wave flattening, PR prolongation, and QRS widening.
