Hx : Fall a heavy object on left hemithorax + chest pain + haemoptysis
O/E : pallor, the pt is in pain, crepitation on Auscultation, SPO2 is 94%, normotensive.
CXR : lung consolidation
The dx : lung contusion

In this dusty weather, a gush of asthmatic patients presented to ER with typical presentation of dry cough and Dyspnea.
Oxygen + salbutamol neubilzer.
Others : aminophylline, HC, or even Mg sulphate.

21 yo male presented with sudden onset severe dyspnea after direct trauma by blunt object.
He was cyanosed but haemodynamically unstable.
O/E distended JVP with absent air entry in left lung field.
Dx and Mx

41 yo male with negative past hx presented to the ER complaining from Abnormal body movement that last for more than 15 min, this movement was in form of tonic Colonic with involuntary passege of urine.
With further hx taking, the pt has the same condition in the past 45 day, at that time all investigations were normal including brain CT, apart from Hb which was 6.
O/E :the pt was disoriented + severly pallor + positive neck stiffness + bilateral babinski response.
CT brain as above.
Hb was 5.
