اذا شفنا كيس Upper git bleeding أو Acute pancreatitis ادز ع تحليل BUN ليش؟ شنو اهميتهن ؟🧐

المريض اجه بـ Moderate diarrhea and vomiting و تگابلوا عليه الناس Flagyl bottle Amikacin vial Zocin syrup و استمر عليهن ٧ ايام، و طلع للبيت. ثاني يوم اجه عنده Severe diarrhea هم استلموا نفس الرجيم اعلاه. و بقى ٧ ايام ثانية و حالته جاي تسوء اكثر و اكثر. بهجي سيناريو، لازم تفكر بـ Antibiotics-induced clostridial infection. و مثل ما تشوفون التحاليل بوزتف. و علاج هجي حالة هي Oral vancomycin (not IV vancomycin). #pediatric

من الشغلات نلاحظها بالطوارئ يجيك بيشنت dizziness or confusion, palpitation, headache, tremor, numbness...etc تركز وياه بالهستري بذات PSHx راح تلكاه مسوي عمليه المعدة كاصها Gastric sleeve هنا خلي بالك نقطتين حتى ما تضيع نفسك أما بسبب - Dumping syndrome تنقسم 👈 إلى early تصير بعد الاكل حوالي بنصف ساعة يصير عنده hyperglycemia نتيجة rapid gastric emptying 👈 أول late تصير حوالي بعد ساعتين نتيجة rapid absorption ويدخل ب hypoglycemia لهذا RBS is a mandatory investigation in the ED كل شي ما تسويله مجرد فهمه الوضع يحتاج اختصاص تغذيه يرتب اله اموره ويقلل الوقت بين الوجبات بدال 3 يسويهن 6 مرات ياكل ويقلل الكمية ..الخ ويعوض الاملاح او يكون سبب - Vitamin deficiency like B12 #emergency

نعتذر ع التاخير بالرد.. لان الكولات ازدحام 💔😅 طبعا هذا advance ECG أول مره اشوفه بالطوارئ 😁 Interpretation of this ECG #limb_leads 👉 Narrow QRS complex regular RR interval rate 75/min with normal axis deviation 👉 Negative P wave of non sinus origin seen in inferior leads particularly lead III with short PR interval + P wave can be seen before QRs complex and after » Accelerated junctional rhythm AJR 👉 Three beats Couplets of PVC is following by compensation pause 👉 ST depression in inferior leads followed by asymmetrical negative T wave » Could be NSTEMI #Pericardial_leads 👉 In pericardial leads there is RBBB pattern + wide regular complex rhythm rate 75/min, after two beats, one PVC seen trigeminy » PVC arising from left ventricle has RBBB pattern 👉 ST depression V2-V6 » Could be NSTEMI Conclusion: >> AJR, RBBB, Coupletes of PVC, Trigeminy >> - NSTEMI can be found - Possiblity digoxin toxicity

قبل لا تركز ع HR لازم تفرق بين اثنين - Stable symptomatic bradycardia - Bradycardic periarrest تعتمد ع vital signs + examination تشوف المريض تعبان باي لحظه يسلمها asystole وانت تنطيه اتروبين ؟!! شي مو منطقي اذا اجاك بيشنت unstable مثل Hypoxia Hypotension Gasping Cold extremities Oliguria.. etc هنا اتعامل وياه ك periarrest اذا دخل asystole انت منطي adrenaline تركيزك يصب ع CPR + monitor اضافة adrenaline هو Alternative to atropine in Mx of bradycardia Bronchodilator Stimulates the entire of myocardium contractility Vasoconstriction Different routes IM ,IV ... كل هاي بجانب المريض اتفيده اما atropine يكون muscarinic antagonist ك effect لا يقارن ويه adrenaline on the heart لهذا تكون aggressive بالتريتمنت ويه periarrest أما اذا كانت البيشنت stable واموره تمام تكدر تمشيه طبيعي ع Mx of bradycardia والي تبدأ ب atropine وتمشي stepwise #emergency

📄 Quick note (VERY IMPORTANT) Normal neutrophil/lymphocyte ratio (NLR) - The NLR is simply the number of neutrophils divided by the number of lymphocytes. - Under physiologic stress, the number of neutrophils increases, while the number of lymphocytes decreases. - The NLR combines both of these changes, making it more sensitive than either alone: »» Stress ==> Endogenous cortisol and catecholamines ==> Elevated NLR - Interpretation of NLR depends on clinical context - NLR elevates rapidly in response to stress (within ~6 hours). This will often be the first indicator of severe illness on the blood count. - Inflammatory disorders may tend to elevate NLR more than non-inflammatory disorders. Thus, a patient with sepsis and an NLR of 15 might not be tremendously ill, whereas a patient with a pulmonary embolism and an NLR of 15 is more worrisome. - The presence of clinical shock with a normal NLR suggests an inadequate cortisol level (i.e. adrenal insufficiency) + The presence of an elevated absolute eosinophil count in a shocked patient should immediately suggest adrenal insufficiency

كيس كان status epilipticus وصينا ع فينتوين من بره هجي اجتنا الأمبولة Crystals in the bottom. This should be discarded لذلك ضروري نتأكد من اللون و من الكرستل قبل اعطاء كل امبولة جديدة.

Phenytoin in digoxin-induced VT. Remember: amiodarone is contraindicated in such cases. #cardio
