Urine of patient with acute post streptococcal glomerulonephritis APsGN اليورن اهنا تقريبا tea-like، و هذا معناها moderate haematuria مرات يكون cola-like يعني طوخ كلش مثل الببسي و هذا معناها severe haematuria و مرات يكون smoky urine يعني افتح من هذا الي بالصورة، و هذا معناها mild haematuria. يجي الطفل عمرة ٦ سنوات مثلا ، و عندة epistaxis صارلة يومين، و مراجع ENT و فاحصين ال PLT count لاكينها طبيعي، فطلعوا الطفل و خلوله packing بس . ثاني يوم الطفل صارل عنده epistaxis اكثر من الاولى و عنده headache و dyspnea و vomiting . اجيت انت (لأن شاطر و تعرف تسوي approach) مباشرة سألتهم عن ال urine amount and color، فكالك دكتور البارحة كله ما صار عندي يورن، و قبل يوم اليورن يشبة لون الجاي . ف الكيس هذا typical of APsGN . Haematuria + oliguria/anuria + hypertension و هنا المريض بسبب الضغط الي عنده، صار عنده epistaxis و acute heart failure و headache. طبعا انا شايف هجي كيس تبكل، بس قديم . #pediatric

Absent thumb in Fanconi Anaemia الفانكوني انيميا هي عبارة عن aplastic anaemia تبدي تقريبا بعمر ال ٣-٤ سنوات، و تتميز بوجود ال skeletal deformities مثل bifid thumb or absent thumb, and hypertelorism. اكيد ال skeletal Symptoms تكون ظاهرة منذ الولادة، بس الأنيميا لا. هذا الكيس كان عنده أيضا Anal agenesis, undescended testes, and hydrocephalus. بالنسبة لل hydrocephalus ما قارية انه يجي وي ال Fanconi anaemia، احتمال incidental، بعد ما اعرف. العلاج الرئيسي هو bone narrow transplant. انا بس ردتكم تشوفون ال absent thumb الي هو bilaterally. #pediatric
