"علاجات تضحك مووو " ع العموم كلناهالها تحتاجين مفراس CT گالت لا، امس مسويه مفراس و هذا هو 👆👆👆 Subcapsular haematoma due to warfarine toxicity يابه سويلنا INR كان 5.6 Warfarine toxicity + minor bleeding = vitamin K 5 mg IV infusion طبعا احنه شارحينها قبل شلون تسوي Reversile of warfarine toxicity تلكوها بالبحث. #hepatology

أجاني رجال عمرة 70 سنة و عندة سكر و ضغط و IHD و قبل مسوية قسطرة. A. History: Cheif complian: diffuse joint pain for the past 3 months. Drug hx: carvedilol 6.25 mg metformine/glineclamide Amiodarone 200 mg Amlodipine/hydrochlorothiazide/Valsartan B. Examination أصعد حجي ع السدية خل نفحصك: Elderly obese male with BP 160/100 Peri-orbital puffness Loss of some denture Bilateral lower limb edema up to half of leg Stage 1 diabetic foot Weak lower limb peripheral pulses No flapping tremor Leukonychia Clear chest to Auscultation CCTA, no added precordial sound C. Ddx on clinical picture (hx and exam) انا شكيت بشغلتين أكثر شي 1. Diabetic nephropathy 2. Amiodarone induced hypothyroidisn AIH D. Investigations: حجي راح اندزك مجموعة من التحاليل : RBS : 362 mg/dl S. Creatinine 0.8 mg/dl و حسبتله الـ CrCl حسب المعادلة CrCl = 140-age/S.Cr 👉👉 CrCl = 87 TSH 0.1 HbA1C 9.8% ECG : atrial flutter with 2:1 AV block اخذت من عنده هستري أكثر تبين أنه عندة severe constipation و كان عندة very hard stool صارلها ٥ أشهر و مراجع عليها و جاي ينطونة فقط Lactulose syrup بدون ما يسألون عن شنو سبب الـ constipation. فتشخصت الحالة مبدئيا على انها Amiodarone induced hypothyroidisn AIH D) Rx: بديت وياه levothyroxine 12.5mcg يوميا قبل الأفطار بساعة كاملة. كلش ضروري تأخذ drug hx من الـ elderly patients لأن غالباً تلكاهم polypharmcy و عدهم هواي مشاكل بسبب الأدوية. #endocrinology

Pediatric oro-pharyngeal candidiasis كلش كمن بالأطفال خاصة بأول سنة، و خاصة بأول شهر. شلون نعالجها ؟؟؟ 1- Nystatin oral drops يجي على شكل 100,000U/ml، و أكو شركات تخلي الـ ml هلى شكل قطرتين و أكو على شكل أربع قطرات، فأنت شوف شنو مكتوب على الباكج مال العلاج A. Pre-mature: 100,000 U (1 cc) 1*4 B. Infants : 2 cc 👉🏽👉🏽 1 cc on each side of the mouth C. More than 1 year: 4-6 cc, half on each of the mouth. تنطي النستانين كل ٦ ساعات، و أذا تنطيه لازم الطفل ما يرضع خلال الـ 10 دقائق الجاية. You continue on nystatin 3 days after disappearance of oral lesions أكثر شي نستعمله هو النستاتين و الريسبوني تصل حتى 90%. 2. Clotrimazole troches 10mg هذا نستعمله للأطفال إلي أعمارهم أكثر من 3 سنوات، يخلي الـ troches بحلگة لحد ما يذوب. يستعمله 5 مرات يوميا لمدة أسبوعين 3. Fluconazole syrup 10 mg/ml In 1st day: give 6mg/kg In next days: give 3mg/kg Continue on treatment for 14 days or 3 days after disappearance of last symptoms. أهم شي تحفظون الرجيم مال النستاتين لأن هو أكثر واحد نستعمله و ما جاي نستعمله صح. #pediatric

منشور دكتورة مريم ذكرني ب ملاحظة كلش مهمة هذا الي بالصورة لـدكتورة مريم. نايس الـ tendinopathy شي سهل بالمقارنة مع الـ aortic aneurysm الـ Fluorquinolones مثل ما جاي تضعف الـ collagen fibers الي موجودة بالـ tendons، أيضاً راح تضعّف الـ proteins الي موجود بـ aorta و تخيّل انت منطيها لشخص عنده hypertension و احتمال عندة mild aneurysm و انطيت وياها Nsaid أو steroids ف هنا كلش راح يزداد الـ risk مال Acute aortic syndromes like aortic dissection or non disseceting aortic aneurysm or even aortic rupture. It is better to avoid Fluorquinolones for elderly hypertensive patients. #pharma

Ventricular bigeminy جاي تشوف Fluctuation between normal and ectopic beat يعني تجي ضربة نورمل و الثانيه ventricular ectopic هاي اسمها ventricular bigeminy. ممكن تتحول إلى VF or VT إذا كان البيشنت عندة hx of IHD أو كان عندة Bifid QRS #ECG

PMHx: HTN ————————————— Dx: Acute coma, pinpoint pupils & absent horizontal eye movement are suggestive of a pontine hemorrhage & can help to differentiate this presentation from classic acute stroke.. ليش يدخل ب coma ؟ لان pons تكون ضمن brainstem وهاي المنطقة بيها reticular activating system هذي المنطقة مسؤوله عconciousness لهذا يدخل بغيبوبة ويعتبر most common presentation وليش صار pinpoint pupils ؟ d2 disrupt descending sympathetic fibres in the pons, resulting in unopposed parasympathetic input It carries a poor prognosis.. #neuro

Blood urea nitrogen BUN/ Creatinine ratio أول شي لازم نعرف شنو الفرق بين الـ BUN و الـ S urea ؟؟ BUN is just nitrogenous components of blood urea. و لأن الـ molecular weight MW لليوريا هو 60، بينما الـ MW للنتروجين هو 28 👈👈 So, the ratio of blood urea to nitrogen is 60/28 = 2.14 If we want to convert S urea to BUN 👉🏽👉🏽 Blood urea / 2.14 أو بصورة إبسط، الـ BUN هي نصف الـ blood urea. Normal S urea is 20-40mg/dl 👉🏽👉🏽 BUN is 10-20 mg/dl Normal S creatinine is 0.6-1.2 mg/dl or 53-106 mmol/L S Cr (mg/dl)= S Cr (mmol/L)/88.4 So, normal BUN/Cr ratio is 5-20 يلا ننتقل للجزء الثاني🚲🚲 Both, urea and creatinine are freely filtered in the renal, but urea can be re-absorbed while creatinine can’t. فلذلك أذا واح يصير عندة renal hypo-perfusion راح يقل الـ GFR، فالـ filtrate راح ينتقل بصورة بطيئة داخل الـ renal tubules فيصير reabsorption لليوريا بينما الكرياتينين ما يصيرله إعادة امتصاص. So NUN is relatively increased with normal or minimal elevation of S Cr 👉🏽👉🏽 elevated BUN/Cr > 20 أذا صار عندة intrinsic renal injury، فراح يصير عندي مشكلة بـ الفلترة، لذلك اليوريا و الكرياتينين أثنينهم يرتفعون و بالتالي تبقى الـ BUN/Cr نورمل. Keep in your mind, if there is acute tubular necrosis ATN (intrinsic renal injury), both renal indices will be elevated but mainly creatinine (urea still be lost via necrotic tubules)👉🏽👉🏽 so this lead to low BUN/Cr ratio with elevated renal indices but mainly Cr. هاي كلش تفيدنا بالاطفال، أذا نريد نعرف هذا الديهادريشن مسبب pre-renal azotaemia لو واصل مرحلة الـ ATN، بطريقة بسيطة جداً. Post renal azotaemia will elevate both indices but mainly BUN (because the tubules are still intact which will re-absorb urea) #nephrology
