🎯 Air Pressure - The Silent Line of Defense in Isolation Rooms
(Air Pressure Integrity. The Invisible Layer of Protection)
Sometimes, risks do not begin with the patient, but with the surrounding air environment.
Saudi national guidance emphasizes that the effectiveness of Airborne Infection Isolation Rooms (AIIRs) depends not only on the room itself,
but also on continuous operational integrity, airflow direction, and regular ventilation monitoring.
🔹 AIIR rooms are designed for patients with airborne infectious diseases such as TB,
and must maintain negative pressure to prevent contaminated air from escaping into adjacent areas.
🔹 National guidance highlights the importance of continuous monitoring of airflow and pressure systems inside isolation rooms to maintain proper airflow direction.
🔹 Frequent or prolonged door opening may compromise pressure stability,
especially when ventilation systems are not properly balanced or ACH levels are inadequate.
🔹 National recommendations also emphasize conducting risk assessments to determine the required number of AIIR rooms based on facility services and anticipated patient risks.
🔹 When an AIIR is not available,
a portable HEPA filtration unit may be used in a private closed room until appropriate placement or transfer is arranged.
💡 In infection prevention, air is never just “empty space”
it is part of a continuously monitored protection system.

في هذا الجزء نستعرض أهم ما تحتاج معرفته عن Fit Test و PAPR لضمان حماية تنفسية آمنة في الممارسة اليومية.
أولاً: Fit Test - التحقق من إحكام قناع N95
يُعتبر Fit Test خطوة أساسية للتأكد من أن قناع N95 Respirator يحقق إغلاقًا محكمًا (Seal) على الوجه، مما يقلل من تسرب الجزيئات المحمولة جوًا.
أنواع الاختبار:
1. Qualitative Fit Test (QLFT):
يعتمد على حاسة التذوق أو الشم باستخدام مواد ذات طعم حلو أو مُرّ.
النتيجة تكون (نجاح/فشل) بناءً على عدم الإحساس بالمادة.
مناسب لاختبار كمامات N95.
2. Quantitative Fit Test (QNFT):
يعتمد على أجهزة متخصصة لقياس نسبة التسرب بشكل رقمي، ويعطي ما يُعرف بـ Fit Factor.
تسلسل الإجراءات (Protocol):
التقييم الطبي والتدريب لضمان جاهزية الممارس.
ارتداء القناع وإجراء User Seal Check للتحقق المبدئي من الإحكام.
بدء الاختبار (حسي في QLFT أو باستخدام جهاز في QNFT).
تنفيذ 7 تمارين مختلفة (مثل التنفس، تحريك الرأس، التحدث) لمدة دقيقة لكل تمرين.
النتيجة:
Pass: يمكن استخدام N95.
Fail: إعادة الاختبار بمقاس مختلف أو استخدام PAPR.
ملاحظة:
يُعاد إجراء الاختبار بشكل دوري (غالبًا سنويًا) أو عند حدوث تغييرات في ملامح الوجه مثل تغيّر الوزن أو العمليات الجراحية.
ثانياً: PAPR - Powered Air-Purifying Respirator
يُعد PAPR خيارًا متقدمًا يوفر مستوى حماية أعلى وراحة أكبر أثناء الاستخدام، حيث يعتمد على تدفق هواء مُرشّح بشكل مستمر.
متى نستخدم PAPR؟
عند وجود شعر في الوجه يمنع إحكام N95.
في حال عدم اجتياز Fit Test.
أثناء الإجراءات عالية الخطورة (Aerosol-generating procedures).
عند الحاجة لاستخدام لفترات طويلة براحة أكبر.
طريقة الاستخدام (BICSL Guidelines):
1. Pre-Use Check:
التأكد من شحن البطارية، تدفق الهواء، حالة الفلتر، وسلامة الجهاز.
2. Assembly & Donning:
تركيب الجهاز (الحزام والمروحة)، توصيل الأنبوب، ثم ارتداء الغطاء (Hood) وضبطه.
3. During Use:
التأكد من استمرار تدفق الهواء ومراقبة مؤشرات الجهاز.
4. Doffing & Cleaning:
بعد الخروج من منطقة الخطر، يتم إيقاف الجهاز وخلعه بحذر، ثم تنظيف وتعقيم الأجزاء القابلة لإعادة الاستخدام، مع الالتزام بنظافة اليدين. | 581 |
