تذكر أن أي مريض تظهر عليه أعراض جلطة دماغية (اعوجاج نصف الوجه ...عدم القدرة على تحريك نصف الجسم .. فقدان قدرة العضلات على الحركة ... فقدان الانعكاسات الحركية ..وجود علامة babinski) فلابد أن تضع قاعدة فى عقلك وهي : ١-لابد من عمل تحليل سكر عشوائي ٢-ممنوع تنزل الضغط مرة واحدة إلا لو أعلى من 220/120 ٣-ممنوع إعطاء أسبرين أو plavix قبل ما تعمل مقطعية ، ليه ؟؟؟؟؟ ٤- ركب كانيولا وحط العيان على جهاز أكسجين. ٥- اطمن على vital signs ٦-اسحب معامل كاملة : CBC ، serum creatinine ، RBS ، ABG ٧-هتطلب مقطعية فورا CT brain ٨- بعد المقطعية، لو مالقتش intracranial hge ، هتعطي للعيان antiplatlet -aspocid أربع أقراص يمضغهم أو يبلعهم -plavix اربع أقراص يبلعهم

Paget's disease of the breast is a type of cancer that resemble an  eczema, with skin changes involving the nipple of the breast. Paget's abscess Is an abscess that recurs at the site of a former abscess which had resolved. Paget's disease of bone commonly known as Paget's disease or osteitis deformans,  is a condition involving cellular remodeling and deformity of one or more bones. These structural changes cause the bone to weaken, which may result in deformity, pain, fracture or arthritis of associated joints. Paget–Schroetter disease is a form of upper extremity deep vein thrombosis (DVT), a medical condition in which blood clots form in the deep veins of the arms. These DVTs typically occur in the axillary and/or subclavian veins. Extramammary Paget’s Disease (EMPD) is a rare and slow-growing malignancy which occurs within the epithelium and accounts for 6.5% of all Paget’s disease.  The clinical presentation of this disease is similar to the characteristics of mammary Paget’s disease (MPD).  Examples, pagets disease of vulva, pagets disease of scrotum.  

To differentiate if the breast lump is cystic or solid   use the US. Fibroadenosis is felt by tips of fingers not by flat hands  because it resemble the normal breast tissue. Lump +breast pain + pain increasing  and decreasing  related to menses = fibroadenosis. #breast_cancer

We now the the common metastatic sites in case of breast cancer 👉 LLBB. Liver and lung (LL) the best ttt of them are chemotherapy. But Bones and brain (BB) the best ttt of them are Radiotherapy. #breast _cancer.

Radiotherapy is used as local therapy. but chemothrapy is used as systemic therapy. #breast_cancer

كلام في محلة وهو يشتغلعلى الاثنين بس تأثيرة ضعيف على الgram positive. كمان المريض اليمني يضبح ويقول الدكتور فاشل عندما يستخدم علاج وبعدين ما ينفعش، وبعدين يغير على علاج ثاني، وقد سمعتها من ناااس كثير وفعلاً المريض مش مسرح تجارب عشان تجرب علية اكثر من نوع، كمان الحالة المادية ما تسمح له انه يستخدم اكثر علاج😊.

Ampicillin is broad spectrum against gram positive and negative لذلك خاصة لو هو صيدلي يبدأ به لأنه ماعندهش نتيجة culture ومايصلح انه يبدأ بحاجات قوية من البداية والمريض لو من البداية نظف القيح كان يمكن ينفعه

السلام عليكم، اكيد ال Ampicillin مضاد حيوي معروف لدى الجميع، بس حبيت انوه على حاجة كثير شفتها وقبل فترة اتى رجل عنده جرح في يده وكلم الصيدلي يدي له مضاد حيوي لانها جالس تقيح علية💔، الصيدلي صرف له ampicillin حبوب 500mg ، المريض يومين وما فيش تحسن؟؟؟ طبعاً الكل عارف ان الampicillin يشتغل على الgram negative ومعظم عدوى اليد gram positive يعني اكيد ما ينفعوه هذا العلاج، وكان المفروض انه يصرف له مضاد حيوي وفقا للمزرعة، ولا اقل شئ لدواء يغطي الgram positiveوالnegativeمثل الAugmentin وطبعا الاخير نفعه كثير وتحسن الحمد لله. نسيت حاجة ضروري قبل اي مضاد حيوي يصرف ان ننتبه اذا المريض عنده حساسية من المضادات الحيوية و اذا عنده مشاكل في الكلية او الكبد او اذا هو يستخدم ادوية اخرى عشان بعص الدوية مفعولها يبطل بسبب المضادات الحيوية او تزيد، وكمان لا تنسوا حامل، رضيع، لانه هناك حاجات خاصة لكل منهم ممنوع تتعامل معهم كاي مريض اخر.
