[APPLICATION FOR LEAVE OF ABSENCE, TERM 1 AY 2023-2024] Please be informed that the deadline for Application for Leave of Absence is extended until November 11, 2023 (Saturday 8:00 PM only) and will follow the updated procedure below. 1. Accomplish LOA Form 2. A copy of the Google Form response will be sent to the student's email 3. Students who are enlisted/enrolled will be dropped/withdrawn, subject to appropriate fees 4. Application will be evaluated 5. The student shall ensure all requirements are submitted 6. Students may be on LOA for one term or more, as there are no limit to the number of terms. 7.
