*Exam Myths vs Facts 1*
(by pace your MRCP – Dr Tanzeel Bukhari)
---
*❌ Myth:*
“Examiners expect a perfect examination every time.”
*✅ Fact:*
Examiners do not want perfection — they want safety, structure, and sense.
A candidate who is calm, systematic, and clinically reasonable scores far higher than someone trying to perform a textbook-perfect exam.
*🧠 Why this myth is dangerous:*
It makes candidates rush through the exam, panic when they forget a step, and miss key findings.
*🩺 What examiners actually reward:*
Logical approach
Clear communication
Patient comfort and safety
Clinical reasoning
Professional behaviour
A safe, structured candidate always beats a fast, flashy one.

*⭐ Dr Bukhari’s Tip of the Day 1 – “Don’t Rush to Diagnose”*
(by pace your MRCP – Dr Tanzeel Bukhari)
In PACES, most candidates fail because they rush into a diagnosis too early — especially in Long Clinical Consultations.
🩺 Your tip:
> “Slow down. Listen first. Diagnose later.”
*Why this matters:*
Patients reveal 80% of the diagnosis in the first minute — if you let them talk.
Examiners reward candidates who show structured thinking, not speed.
A calm, paced approach reduces errors and boosts your score in clinical judgement, communication, and patient welfare.
*💡 Practical takeaway:*
In every consultation, let the patient complete their opening story without interruption — you’ll get cleaner clues and make smarter decisions.

*💊 Drug of the Day 1 – Bisoprolol*

*🧠 Mechanism*
Selective β1-blocker → slows heart rate, reduces myocardial oxygen demand, improves ventricular filling.
*🔍 Indications*
Heart failure with reduced EF
Stable angina
Rate control in atrial fibrillation
Hypertension
*⚠️ Key Safety Points*
Start low and uptitrate slowly (especially in heart failure).
Avoid in severe asthma, bradycardia, or AV block.
Monitor HR, BP, and symptoms of decompensation.
*❗ Common Side Effects*
Fatigue & dizziness
Bradycardia
Cold extremities
Sleep disturbance or vivid dreams
Worsening asthma symptoms (rare but important)

🩺 *Clinical Snapshot 1 – Clubbing*
(by pace your MRCP – Dr Tanzeel Bukhari)
*🔍 What You See*
Bulbous enlargement of the distal fingers
Loss of nail angle
Increased nail curvature (“parrot beak” appearance)
*🧠 Clinical Meaning*
Clubbing indicates chronic hypoxia, chronic inflammation, or proliferative conditions.
Think of serious underlying causes rather than benign explanations.
*🩻 Most Important Associations*
*Respiratory* : Lung cancer, bronchiectasis, pulmonary fibrosis
*Cardiac* : Cyanotic congenital heart disease, infective endocarditis
*GI* : Cirrhosis, inflammatory bowel disease

🔬 Pathogenesis Simplified 1 – Asthma
(by pace your MRCP – Dr Tanzeel Bukhari)
1️⃣ Airway Inflammation
Allergens/irritants trigger Th2-mediated inflammation, causing eosinophil recruitment → airway wall swelling.
2️⃣ Bronchial Hyper-responsiveness
Inflamed airways become over-reactive, excessively constricting to cold air, exercise, or allergens.
3️⃣ Reversible Bronchoconstriction
Smooth muscle constriction narrows the airways → wheeze, dyspnoea, cough — reverses with bronchodilators.
4️⃣ Mucus Plugging
Goblet cell hyperplasia increases mucus secretion → airflow obstruction, especially in severe attacks.

🚨 *Red Flags Series 1 – Headache*
(by pace your MRCP – Dr Tanzeel Bukhari)
1️⃣ *Sudden “thunderclap” onset*
→ Think subarachnoid haemorrhage — requires urgent CT ± LP.
2️⃣ *Headache with fever + neck stiffness*
→ Suggests meningitis/encephalitis — urgent antibiotics after cultures.
3️⃣ *Headache with papilloedema or new neurological deficit*
→ Indicates raised intracranial pressure — avoid LP, urgent neuroimaging.
