#ملاحظة_طوارئ ⚠️ Sepsis رغم أهمية الموضوع ماكو تعريف ثابت ومتفق عليه واكو عبارة مذكورة (Although sepsis was described over 2,000 years ago, clinicians still struggle to define it, and there is no 'gold standard.) 🟢 بصورة عامة حتى تميز كيسات ال Sepsis بالطوارئ قبل تعلمنا على Criteria of SIRS بالجديد صار عدنا شي اسمه Sofa score 👉 The quickSOFA is a bedside prompt that may identify patients with suspected infection who are at greater risk for a poor outcome outside the intensive care unit (ICU) ♦️ Criteria : 1️⃣ RR > 22 2️⃣ Altered mentation (GCS < 15) 3️⃣ SBP < 100 اذا لكينا نقطتين عند المريض هذا High risk of mortality

#ملاحظة_طوارئ 🟠 شوكت تكول هذا المريض دخل ب AKI ؟ 1️⃣ اذا الكرياتين ارتفع أكثر من 0.3 mg/dl خلال أقل من يومين 2️⃣ اذا الكريات+1
#ملاحظة_طوارئ 🟠 شوكت تكول هذا المريض دخل ب AKI ؟ 1️⃣ اذا الكرياتين ارتفع أكثر من 0.3 mg/dl خلال أقل من يومين 2️⃣ اذا الكرياتين ارتفع أكثر من 1.5 مرة خلال اسبوع 3️⃣ اذا ال Urine أقل من 0.5 ml/kg/hr خلال 6 ساعات 👍اي وحدة من ذني تصير معناهه المريض دخل ب AKI يعني إعتمادنا فقط على الكرياتين وال UOP .. ✋ هنا حتى تكول هذا AKI.. لازم تتأكد المريض اصلا ما عنده CKD ولازم تعرف ال Baseline S. Creatinine ✅ ليش مهم تعرف هالشي ؟ ممكن معرفتك بالبداية تنقذ المريض وتمنع تحوله الى CKD ويبقى يغسل طول عمره ▶️ Mostly cause is Pre_renal So > 🔸good hydration 🔸Rx of underlying cause 🔸Stop Offending drugs (like NSAIDs, ACE) 🟥 زين اذا احتاريت هذا المريض هو AKI لو CKD شلون تميز ؟ 👉 Abd US > Small kidneys, Poor CMD Suggest CKD 👉 Lab > S.Ca, PTH, Hb ....

#ملاحظة_طوارئ 🟥 موضوع مهم جدا .. Shock = Circulatory failure ( الأوكسجين اللي يوصل للخلايا أقل من إحتياجها) 🤔 لو سألنا السؤال التالي : مريضين عندك 1️⃣ pt A > BP 110/40 2️⃣ pt B > BP 90/60 منو عنده Better tissue perfusion ? للوهلة الأولى تعتقد إنه المريض A وضعه أحسن بس بالحقيقة لا 🟠 من تريد تحدد المريض داخل بحالة Shock أو لا .. تعتمد على حساب MAP Mean arterial pressure (( MAP = SBP + DBP + DBP /3 )) ♦️الضغط الإنبساطي أهم من الضغط الانقباضي pt A > MAP = 63 pt B > MAP = 70 ال MAP فوق ال 65 عادة يكون كافي حتى يحافظ على Tissue perfusion ___ 🟠 Hypotension لا يساوي Shock مو كل هبوط بالضغط معناه المريض بحالة Shock 👉 3 markers of tissue hypoperfusion 1️⃣ Metabolic acidosis 2️⃣ High Lactate 3️⃣ Reduced UOP ___ ✅ العلاج المبكر ينقذ حياة المريض.. Every minute of hypoperfusion = more infarction of the Viral Organs شلون تشخص الأسباب وتتعامل وي الحالة والعلاج المناسب في المحاضرة التالية

Degludec has a much higher half-life than Detemir and therefore maintains a basal insulin level when the patient omits or forgets doses. This can prevent DKA.
