April 30 - May 6. São Paulo, Brazil. Deep Plane Brazil. It was a great honor to be invited as a speaker and cadaver course trainer at one of the most intensive international events dedicated to facial plastic surgery. 6 days of continuous education: 4 days of lectures and live surgery 2 days of cadaver lab More than 300 participants from around the world, outstanding organization, modern technologies, endoscopy, deep plane facelift, live surgery, and anatomical dissections. During the conference, 10 live surgeries were performed. On one of the days, there were 4 live surgeries in a row. A huge hall with a large screen, real-time surgical broadcasting, a cadaver lab with endoscopes and full equipment at every table, and many doctors and speakers working directly with the participants. This was a truly impressive level of international education, experience exchange, and surgical energy.

New York 2026 🏙️🏆 April 23-26, I had the honor of speaking at the New York Facelift and Rhinoplasty Master Course organized by Ramtin Kassir. An incredible atmosphere, live surgeries, cadaver dissections, lectures, and communication with surgeons from all over the world - this is what makes such events truly special. I presented my lecture on rhinoplasty, and together with my friend Fas Arshad we received the awards for Best Innovative Technology 🏆 But the most important reward is always knowledge, experience, and the feeling of being part of one great international surgical family. Rhinoplasty. 2-year result. ✨ Case with previous trauma – the patient had a childhood scar on the nose and columella, with damaged cartilage. Preoperative CT was essential to understand the anatomy and plan the strategy. Surgery was performed using a closed approach: – osteotomies with preservation of the keystone area – let-down technique (high strip) – lowering of the dorsum as one unit – reconstruction of the tip with additional cartilage No open approach – preserving tissues and ensuring a more natural recovery. 2 years later – a stable, harmonious, and natural result. ⭕️ KAMINSKYI

Interesting finding during dissection. While working on the deep plane technique in the cadaver lab, we discovered cosmetic threads previously placed in the face. This is how threads placed by a cosmetologist can look in reality. You can clearly see how close they are to critical anatomical structures and how irregularly they pass through different facial layers. This is not only technically incorrect – it can be dangerous.

Real Beauty. Kate Hudson is a wonderful actress, but how she looks in real life and how she appears in advertising campaigns are not the same. Girls, women - remember that true beauty is natural. A surgeon can move tissues, reposition them, reshape, or remove what needs to be removed, but the biggest “improvements” you see in many images often come from retouching, artificial intelligence, and Photoshop. The correction was based on an asymmetrical resection of the bony dorsum, followed by division of the upper septum in its bony and cartilaginous parts to fully mobilize the entire osseocartilaginous pyramid. After that, the nasal pyramid was repositioned, stabilized, and the tip was reshaped and symmetrized to improve structure, contour, and overall balance. Blepharoplasty was also performed, which helped refresh the eye area while maintaining a natural look. Five years later, we can see not only a stable surgical result, but also how well the face has matured over time. The patient looks harmonious, feminine, and even more beautiful today, which is also an important reminder that age itself does not always take away beauty - very often, it refines it. We all age differently - and aging is never driven by a single factor. Genetics plays a major role, but it is only one part of the picture. Lifestyle, nutrition, ethnicity, skin quality, anatomy, and your individual aging pattern all influence how the face changes over time. That is why two people of the same age can look completely different. Surgery can do a lot. It can refine, reposition, restore proportions, and improve details. But even the best targeted surgery cannot completely change the foundation you were born with or turn one face into another. That is why, when people look at someone else’s transformation and hope to age or change in exactly the same way, it is important to remember that the comparison is never fully correct. Different tissues, different structure, different biology, different response to treatment. Aesthetic medicine is not about copying someone else’s result. It is about understanding your own starting point and choosing the right strategy for your face. The best outcomes happen when expectations meet reality - and when beauty is approached with intelligence, not illusion.

Capsular contracture - a complication we discuss often, but still do not classify perfectly. After breast implant placement, the body normally forms a thin capsule around the implant. This is a natural reaction. Problems begin when this capsule thickens, tightens, and starts to compress the implant, changing the shape, firmness, and sometimes causing pain. Many still describe capsular contracture using the Baker classification: Grade I - soft and natural Grade II - slightly firm Grade III - firm and visibly distorted Grade IV - hard, painful, and deformed But an important nuance is often missed: there is still no truly precise and universally accepted classification of capsular contracture. Yes, many refer to the Baker scale, but Baker himself did not give a fully strict, objective, measurable description. In practice, this scale is based largely on clinical impression - how the breast looks and feels. That means different surgeons may assess the same case differently. For patients, the main point is simple: not every capsule is a problem, but when the capsule becomes thick, tight, painful, or distorts the breast, it requires proper evaluation and sometimes treatment.

Columbus Clinic in Milan is one of those places I always want to photograph 📸 It is where I work, and every time I arrive, it catches my eye in a new way. Especially in spring, when the garden, the courtyard, and the light completely change the atmosphere. At the heart of Columbus is Villa Romeo - a Liberty-style residence designed by Giuseppe Sommaruga, later connected to Nicola Romeo of Alfa Romeo. After the Second World War, the newer clinic buildings were developed through the studio of Gio Ponti. Inside, Columbus is modern, elegant, and highly functional - a place where medicine, aesthetics, and architecture meet naturally. And the final touch is Bastianello - a historic Milanese name since 1950, bringing warmth, character, and a special atmosphere to the clinic.

Swelling after rhinoplasty is a normal and expected part of healing. This is an example of swelling two weeks after surgery. A revision rhinoplasty was performed without rib cartilage using a technique of repositioning the osteo-cartilaginous nasal dorsum pyramid. The anterior part of the septum had been removed during a previous surgery, which resulted in reduced tip stability. To avoid widening the nose and to eliminate the need for rib cartilage - while focusing on hump reduction, improving symmetry, and enhancing breathing - a closed approach with structural repositioning was chosen. It is important to understand that swelling does not occur in the cartilage or bones. Swelling primarily affects the skin and soft tissues. This is why postoperative care should focus not only on surgical healing but also on skin management. Proper rehabilitation and consistent work with the skin help swelling resolve faster and improve the final result. The patient is currently following a rehabilitation protocol and actively working on skin recovery.
