Complication VS Sequela

🔸 Complication (مضاعفات): A new medical problem that develops during the course of a disease or as a result of it. 🔸 Sequela (آثار وعواقب متبقية): A residual effect or long-term consequence that remains after the original disease or acute condition has resolved. • Complication = An additional problem. • Sequela = A lasting consequence. ● Example in Meningitis: - ↑ICP, Hydrocephalus (Complication). - Hearing loss after recovery (Sequela).

🔹 Why do we wait 30sec during Dix-Hallpike Test? To detect the latency of nystagmus. In Posterior canal BPPV: • Otoconia move within the semicircular canal. • Abnormal endolymph movement stimulates vestibular receptors. • Vertigo and nystagmus occur. →If central vertigo = rapid ,No latency period. →If peripheral vertigo = No rapid, have latency period; so we wait for 30sec⭐️. 💡Typical finding = Up-beating + torsional nystagmus. #ENT

🔹 Lateral Semicircular Canal BPPV.. Dx test: Supine Roll Test (Pagnini–McClure Test). Tx maneuvers: ◈Geotropic: • Lempert maneuver (Barbecue Roll). • Gufoni maneuver. ◈Apogeotropic: • Modified Gufoni (Appiani maneuver). 💡Horizontal nystagmus during the Supine Roll Test suggests lateral canal BPPV. #ENT

🔹 Adenoid Hypertrophy

Clemens & McMurray Classification

Based on the percentage of Choanal obstruction: • Grade I: Up to 1/3. • Grade II: 1/3 – 2/3. • Grade III: 2/3 to nearly complete obstruction. • Grade IV: Complete obstruction. أيش الاختلاف عن Cassano؟ - Clemens & McMurray: يعتمد على نسبة امتلاء الـ Choanae إلى ارتفاعها. - Cassano: يعتمد على نسبة انسداد الـ Choanae. ←التصنيفان متقاربان فِ الفكرة، بس مَ نعتبرش درجاتهم متطابقة تماماً. #ENT

🔸 Ear Packing Types: • Aural pack: General term for ear packing. • BIPP pack: Bismuth Iodoform Paraffin Paste. • Ear wick: Helps deliver topical medication in severe Otitis externa with canal swelling (edematous). • Vaseline-impregnated Ribbon Gauze Ear Pack: Sterile ribbon gauze impregnated with Vaseline, used for ear packing. #ENT

🔹 Types of Nasal Packing in Epistaxis →Anterior packing: • Ribbon gauze. • Merocel. • Rapid Rhino. →Posterior packing: • Foley catheter. • Bellocq's pack. 💡Anterior epistaxis is more common than posterior epistaxis. #ENT

🔹 Tonsillar Hypertrophy Brodsky Grading (score) • Grade 0: Absent tonsils. • Grade 1+: <25% obstruction. • Grade 2+: 25–50%. • Grade 3+: 50–75%. • Grade 4+: >75%. 💡 Golden point: Grade 4+ = Kissing tonsils. - Note: This grades tonsil size, NOT the severity of tonsillitis. #ENT

Cassano Grading

Hydralazine

🔹 MOA: Direct arteriolar vasodilator → ↓ SVR → ↓ BP. 🔹 Uses: Severe HTN, especially in pregnancy. 🔹 Adverse effects: Reflex tachycardia, headache, fluid retention. ❗️ Serious adverse effect: Drug-induced lupus-like syndrome. #pharmacology
