1⃣ Personal data:
1. Name of patient
2. Age of patient or date of birth
3. Gender
4. Residence and origin
5. Tribe
6. Date of admission
7. Informant
2⃣ Main Complains C/O:
The main complaint of patient and duration should be mentioned
according to the chronicity.
3⃣ History of present illness (HPI) and this consist of:
1. Analysis of the main complaint
2. Symptoms of the affected system
3. Systemic review we start from above
🔹Central nervous system
symptoms of abnormal higher function symptoms of cranial
nerves affection Symptoms of abnormal motor system
Symptoms of abnormal sensory system.
🔹Cardiopulmonary system
Cough, cyanosis, shortness of breath, palpitations, chest pain, syncopal attacks, lower limbs swelling.
🔹Gastrointestinal tract
Dysphagia, heart burn, abdominal pain, abdominal distention, loss
of appetite, loss of weight, jaundice, bowel habits, lower limbs or
generalized body swelling.
🔹Genitourinary system
Ask about urine amount, frequency, color, and burning micturition
Ask about loin pain
🔹Musculoskeletal system
Ask about joint pain and/or swelling
Skin rash Hypo or hyperpigmentation
4⃣ Past medical history (PMH):
☑️Past history of similar condition
☑️Past history hospital admission if positive ask about level of admission (general words, HDU, ICU, ….), duration of admission,surgical operation, or blood transfusion
☑️History of chronic disease
5⃣ Developmental history:
👶Prenatal history:
🤰1st trimester ask about drugs, fever with skin rash, radiation
🤰2nd trimester ask about vaccination, sonography for congenital malformation, gestational DM, pregnancy induced Hypertension.
🤰3rd trimester ask about PIH, GDM, vaginal discharge, drainage of
liquor, APH, maternal fever or infection
Natal history: ask about gestational age, site of delivery, mode of
delivery, and if abnormal labor which type induced, instrumental, emergency caesarian section or elective, and what was the indication.
🧒Postnatal history:
Ask if the patient cried immediately or not, if not cried immediately what type of resuscitation done and for how long? Passed meconium and urine within the 1st 24 hours or not?
Started breast feeding immediately or not?
Any history of neonatal jaundice, convulsions or admission to
NICU?
👼Milestone:
The proper assessment of newborn immediately post-delivery
and monitoring should be done by APGAR score.
Then the development followed up by milestone, we have different areas to assess development:
1. Growth motor skills
2. Fine motor skills
3. Social skills
4. Vision
5. Hearing and speech
6⃣ Family history:
🔹Ask about family history of similar condition
🔹Ask about family history of chronic disease
🔹Ask about family history of neonatal or early infant deaths.
🔹Ask about consanguinity between parents
🔹Draw the family pedigree
7⃣ Drug history:
🔹Ask if the patient on long term medication?
🔹If he/she known to be allergic to any drug?
8⃣ Social history:
Ask about socioeconomic status & income, Housing condition, number of rooms, ventilation, sanitation, safe water supply, electrical supply.
Impact of disease in the patient and family,Health insurance.
Animals, Birds, or gardens at home
Bad habits smoking, alcohol, or addiction.
9⃣ Summary of history (focus history)
🔮Positive findings (support your provisional diagnosis) + importrant negative findings (to exclude other deferential diagnosis)

🔥بداية بمكتبة كلوك وايز في مدني حي المنيرة جوار مجمع البركات تجيهم طوالي بحولو ليك والله المكتبة دي فتحت بابها للناس ومستعدين ينقلو الملفات لي اي شخص تقريبا في اسبوع قرابة ال ٧٠ شخص نقلو الملف ما شاء الله عليهم شكرا عبدالله ومحمود والشباب المعاهم
🔥بقية الشباب في الولايات برضو ما قصروا ورسلو لي ارقامهم بحيث اي زول محتاج هم بحولو ليه 🌹
