Some antibiotics have low or unreliable urinary exposure, which are mainly eliminated by other routes, or are not clinically recommended for routine UTI treatment.
❌ #Tigecycline : Low urinary exposure; only about 22% is excreted unchanged in urine.
❌ #Minocycline & #Doxycycline : Relatively low urinary recovery or excreted changed in Urine.
❌ #Macrolides ( #Erythromycin – #Clarithromycin – #Azithromycin) : They are not routine antibiotics for typical bacterial UTI as it is excreted via hepatobiliary route only.
❌ #Clindamycin : same as Macrolides.
❌ #Moxifloxacin : unlike other Fluoroquinolones, Moxifloxacin is bile excreted.
❌ Oral #Vancomycin : Very poorly systemically absorbed, so it cannot provide reliable urinary concentrations Nor treating systemic infections via oral route.

Antibiotics that are used/reported only for urine culture isolates and should not be reported for isolates from other specimen types:
☄️ Norfloxacin
☄️ Nitrofurantoin
☄️ Fosfomycin with E. coli and E. faecalis
☄️ Amikacin with Pseudomonas aeruginosa
☄️ Ciprofloxacin with Enterococci in uncomplicated cystitis
☄️ Cefazolin in cases of non-ESBL-producing E. coli, Klebsiella pneumoniae, and Proteus mirabilis in uncomplicated cystitis.
(in this case Cefazolin predicts susceptibility to Cefaclor – Cefdinir – Cefpodoxime – Cefprozil – Cefuroxime – Cephalexin – Loracarbef)
