Sporadic Gastrinomas: Most commonly located in the duodenum (60-80%). Typically small (<5 mm) in size. Can be multiple. MEN 1-Associated Gastrinomas: All tumors are located in the duodenum. Overall Location: The vast majority (approximately 90%) of gastrinomas occur within the 'gastrinoma triangle'. The gastrinoma triangle is bordered by: The junction of the neck and body of the pancreas medially. The junction of the second and third parts of the duodenum inferiorly. The junction of the cystic and common bile ducts superiorly. Tumor Grading: Gastrinomas are graded according to WHO NET criteria. General Progression and Survival: The progression of gastrinomas is generally relatively slow. The overall 5-year survival rate is 65%. The overall 10-year survival rate is 51%. Impact of Complete Resection: Patients with complete tumor resection have excellent 5- and 10-year survival rates (90-100%). Prognostic Influence of Tumor Location: Patients with pancreatic tumors have a worse prognosis than those with primary tumors in the duodenum. Predictive Markers: There is no established marker to predict the biological behavior of gastrinoma.
