Semen analysis Macroscopic : 1- liquefaction time : Immediate clotting of semen is due to elements of seminal vesicles then it will liquified after 5-20min which is due to proteolytic enzymes of prostate, so prolonged LT (>60min) indicate prostate hypo-function 2- semen viscosity : Increase viscosity indicate seminal vesicles problems, and lead to impairment of sperm motility 3- appearance — Whitish color indicate high sperm numbers or increase WBC — yellowish : pyospermia — reddish : hemospermia 4- volume : should not less than 1.5ml . — low volume : improper collection, retrograde ejaculation, ejaculatory duct obstruction . — high volume : prolonged abstinence, urine contamination, or infection — if there is no semen, the condition called aspermia 5- semen PH Should be alkaline (7.2) Microscopic 1) count Sperm count should not be less than 15million per ml and not less than 39millions per ejaculate . If the ejaculate was 2ml and sperm count is 17million per ml , this is abnormal even if the count was normal per ml. If the count less than 15 M, the condition called oligospermia . If there is no sperms, the condition called azoospermia (always think about post testicular causes) 2) motility Two types of motility, progressive and non- Progressive . Progressive sperms should not less than 32% per total number of sperms . Progressive sperms + Non progressive should not less than 40% . If the total motility less than 40% or progressive motility less than 32%, the condition is called asthenozoospermia 3) morphology : Normal shaped sperms should not less than 4% of total sperms, if it was less the condition called teratozoospermia 4) vitality Should not less than 58% of total sperms 5) round cell counts : Should not more than 5 cells/HPF
