Viral haemorrhagic fever VHF : 1. Group of viral infection that characterized by multi system affection with high predilection to the blood vessles. 2. The implicated one in Thi-Qar is Crimean Congo CCHF. CCHF outbreaks have a case fatality rate of up to 40%. The virus is primarily transmitted to people from ticks and livestock animals. Human-to-human transmission can occur resulting from close contact with the blood, secretions, organs or other bodily fluids of infected persons. 3. Incubation period : 1-13 days, according to mode of acquistion of the virus. 4. C. P constituanal symptoms + specific symptoms which include cutaneous, orificeal, and internal bleeding. 5. Rx .. Supportive .. The antiviral drug ribavirin has been used to treat CCHF infection with apparent benefit. Both oral and intravenous formulations seem to be effective. #infectious_disease

Congenital adrenal hyperplasia CAH (21 hydroxylase deficiency) A) clinical picture 1- Sx of glucocortiocoid deficiency — hypoglycaemia : fainting attacks, diaphoresis, focal neurological deficit, heart failure, and seizures (misdiagnosed as epilepsy) — hypotension : is marked — CBC show eosinophilia and lymphocytosis with relative increase RBC count due to dehydration 2- Sx of mineralocorticoid deficiency — hyperkalaemia : muscle weakness, sudden cardiac death — salt-losing : polyuria, hypotension, seizure , cardiac arrhythmia, and hypotonia — acidosis : abdominal pain, reject feeding, vomiting, and diarrhea — signs of dehydration : depressed fontanelles, sunken eye, delayed skin turgor, reduce urine output, increase in renal indices(AKI), delayed capillary refill, dry tongue [abdominal pain + vomiting+ polyuria+ acidotic breathing will be misdiagnosed initially as DKA) 3- Sx of androgen excess : ambiguous genitalia in females, macropenis in males 4- Signs of ACTH excess : progressive darkening of the skins and mucous membrane 5) since it is AR disorder , so positive consanguinity is present B) investigations — serum 17 hydroxyprogesterone is elevated — ACTH challenge test will show increase in OHP — hypoaldosteronism with its consequences (hypo Na, hyper K, acidosis) — US may show female gonads , CT show adrenal enlargment. C) treatment : — glucocorticoid replacement therapy by oral HC 15-20mg/m2/day lifelong in 3 divided doses — mineralocorticoid replacement by fluorohydrocortisone 0.05-0.3mg/day — plastic surgery for females with clitoromegaly and treatment of precocious pseudo puberty in males #pediatric

هلوو هاي يسمونها erythema ab igne، كلش كمن بسبب ال heat therapy الي جاي يستعمل. اشلون الحرارة تخلي الجلد هجي، محد يعرف، بس يكولون عن طريق Vasodilitation and melamin stimulation. علاجها، هو تتجنب الحرارة اكيد يعني 😐😐. اكو نسبة قليلة من الحالات ممكن تتحول إلى squmaous cell carcinoma SCC.

كيييس من العمر، ما يتكرر بعددد 😍😍😍. إذا ما تعرفه فهذا شي نورمل. اما اذا تعرفة فهو هذا الابنورمل 😑😑 Since birth + unexplained progressive deterioration of renal function. Bilateral knee pain (just mild pain). Otherwise, he is in good health. Dx @AASmedicalbot #pediatric
