[COVID-19 Vaccination Data Survey for DLSU Students] Hello, Lasallians! You are invited to participate in the University-wide survey on the COVID-19 Vaccination Data of DLSU Students in compliance with the Commission on Higher Education (CHED) Memorandum from the Office of the Executive Director No. 193, Series 2021 dated October 01, 2021, in line with the implementation of the National COVID-19 Vaccination Program. The data obtained for this survey will also be used to inform our plans for the gradual resumption of in-person classes.
