*Few pearls for Vasovagal syncope explanation* Occurs when you faint because your body overreacts to certain triggers, such as the sight of blood or extreme emotional distress. It may also be called neurocardiogenic syncope. The vasovagal syncope trigger causes your heart rate and blood pressure to drop suddenly. That leads to reduced blood flow to your brain, causing you to briefly lose consciousness. Vasovagal syncope is usually harmless and requires no treatment. But it's possible that you may injure yourself during a vasovagal syncope episode Good Luck

*Few points about primary biliary cholangitis* is considered an autoimmune disease, which means your body's immune system is mistakenly attacking healthy cells.in this condition bile ducts which are slender tubes that carry the digestive fluid bile are destroyed. Bile ducts connect your liver to your gallbladder and to your small intestine. slender tubes (bile ducts) carry the digestive fluid bile. bile can back up in your liver and sometimes lead to irreversible scarring of liver tissue (cirrhosis). *Few golden points in Cardiology :* Never rush to lsn murmur while examination of precordium....always make your propable diagnosis by picking up peripheral signs related to that murmur and then put stethoscope on chest to confirm that murmur associated with those peripheral signs.......In practice we all go panic to lsn murmur and never correlate that murmur with peripheral signs and so loose the station easily...........................like e.g. 1. Low volume radial pulse+Loud S1+tapping Apex =Mitral stenosis. 2. Low volume radial+Soft S2+Heaving Apex= Aortic stenosis. 3. Good volume pulse+Displaced Apex+Soft S1= Mitral Regurgitation..... So while going hosp always practice to pick findings in pulse Apex and S1 S2.....it ll clear the confusion to great extent....then put stethoscope on chest to correlate murmur with findings..... In General Stenotic lesion give calm n peace look to precordium and whereas Regurgitation lesions give hyperdynamic look to precordium on inspection.... In pure Stenotic lesions Apex is nondisplaced.......in pure Regurgitation lesions Apex will be usually displaced with hyperdynamic look to precordium.. In mixed lesions position of the Apex or Peripheral pulsations will decide predominant lesion clinically........... Murmur of one valve may radiate to other areas aswell...like e.g AS murmur may radiate all over precordium so insuch situations see where murmur has maximal intensity n see character of murmur is same over precordium or not..... In young age Bicuspid is cause and in old age Calcification is the cause Whenever there is Regurgitation of valve then alwaz divide the causes into Acute Vs Chronic......acute AR Vs Chronic AR......acute MR Vs Chronic MR .Whenever there is Mitral stenosis the cause is Rheumatic Rheumatic and Rheumatic......dats it....90% MS is Rheumatic Causes of acute AR? 1. Aortic dissection. 2. Aortic root dilatation. 3. Acute I.E Causes of acute MR? 1. Ruptured cordae tendanea 2. Post myocardial infarction 3. Acute I.E Put Rheumatic in any chronic lesion of valve... Although acute Rheumatic fever may cause valve regurgitation Good Luck

The word INFECTION is tolerable ( not considered as a Jargon by most of the examiners) so u will say to your patient that you have bug infection ( in your lungs, tummy whatever etc) Good luck.
