History of Present Illness
A 28-year-old male presents to the outpatient clinic complaining of severe, progressive fatigue and dyspnea on exertion over the past three months.
Upon further questioning, he reports a prolonged history of chronic diarrhea and unintentional weight loss.
Recently, he has also started experiencing abnormal neurological symptoms, specifically a "pins and needles" sensation and numbness in his lower extremities.
Physical Examination
The patient appears pale and lethargic. Abdominal examination is notable for a distinct, palpable mass in the right iliac fossa.
Neurological examination confirms peripheral neuropathy, with a noticeable decrease in proprioception and vibratory sense in the bilateral lower limbs.
Diagnostic Interventions
Given the chronic gastrointestinal symptoms and the abdominal mass, a colonoscopy is performed.
Tissue biopsies are obtained, and subsequent histopathology reveals the presence of non-caseating granulomas.
